Question 1a. If you are not a citizen of the United States, please check the "No" box for Question 1a and return the form.
Question 1b. If you are no longer a resident of Anne Arundel County, check the "No" box for Question 1b, indicate your new address at the bottom and return the form. If you do not have the summons in your possession, please fax us a copy of your drivers license or any type of bill with your current, out of county resident address information. Question 4. If you are over the age of 70 and do not wish to serve, check the "Yes" box for Question #4 and return the form.
Question 8. If you have served as a juror in Anne Arundel County within the last three years and want an exemption from Jury Service at this time, check the "Yes" box for question 7, provide the date of your service and return the form.
Question 9. If you are active duty military and want an exemption from Jury Service at this time, check the "Yes" box for question 8, attach a copy of your military ID and return the form.
Question 10. If you have any type of hardship - as indicated in Question #10, you must attach a letter detailing your situation and we must receive your letter within 10 days. The letter alone does not excuse you from Jury Duty - only a Judge can do that. Once a Judge has made a decision, you will be notified by mail. Any person asking to be excused for medical reasons must provide the Jury Office with a note from your doctor stating that you cannot serve as a juror due to medical reasons.
